NCLC Renewal Applications, and Contact Update Form
CERTIFICATION RENEWAL APPLICATION
Click on the link above to download the renewal application form and instructions. Certifications are due for renewals every three years. Return your renewal application with the supporting documents of continuing education (read the Certification Handbook) and the $95 payment with the application. Make checks payable to National Council on Laser Certification. -- or if you wish to pay by credit card you can complete that information on the application form.
Once you've completed the renewal application, you can pay the fee online by

NOTE ON AMNESTY PROGRAM FOR LAPSED CERTIFICATIONS:
Due to a variety of factors in 2020 that have been beyond the control of Certified Individuals, many have allowed their Certifications to become inactive. Effective July 2020, our Amnesty program has been extended through Sept 30th of 2021 and allows those with expired Certifications to renew them with no penalties nor requirement for re-examination, by simply completing the renewal form and paying the regular $95 renewal fee which is good for three years.
